New Patient Paperwork Made Easy

We use secure electronic paperwork for all new patients. After your appointment is scheduled, you’ll receive two separate emails. It may take up to an hour to receive both emails.

1. Welcome Email

  • Includes important information and your temporary password

  • The password allows you to access your patient forms in the CareToMe Patient Portal

  • You’ll be asked to create a new password when you log in

2. Patient Portal Email

  • Contains your new patient forms

  • Forms can be completed digitally before your appointment

Can’t Find the Emails?

  • Check your Spam or Junk folder — sometimes both emails end up there.

  • If you still can’t find them or are having trouble, please call the office where your appointment is scheduled:

If you’re unable to complete your paperwork before your visit, please arrive 30 minutes early. Our staff will be happy to help you fill out your forms electronically in the office.
